A drunk gentleman came in to ED and asked me "How much for a massage and handjob"?

I was working the Saturday night shift in ED. This older man comes in and asks me really loudly "How much for a massage and a handjob?" Then everyone started laughing! Including the senior doctor. I went bright red... I know he was drunk but what did he mean by that? Was I dressed like a prostitute or something? Were they laughing at me or him? I'm an intern by the way.
